No. Every item on the Comfort Menu is included in your appointment, however many you pick.
Is this a booking? Am I confirmed?
Not yet — this form tells us you would like to come in and how to make you comfortable. Someone from the clinic will call you to confirm the exact date and time.
Who can see what I write?
Only the clinical team treating you. Your answers are stored on our own secure server and are never sold, shared or used for advertising.
I am genuinely terrified of the dentist. Is that a problem?
It is the opposite of a problem — it is exactly why this form exists. Tell us on the “How you feel” step and we will slow the appointment down, explain each step before it happens, and stop the moment you signal.
Can I change my choices later?
